Make sure you arrange help before you leave hospital if you know you will need some assistance for a few weeks after surgery etc.
Talk to the discharge nurse or hospital social worker before you are discharged if you may need help with shopping, food prep, personal care, transport to medical appointments. Compack packages provide six weeks of funded support for eligible people following hospital discharge.
NACRS partner with Care Connect to deliver services to people in receipt of Compack packages – but this must be arranged before you leave hospital.
